In Without, the young woman named Joslyn is hired as the caretaker for Frank, an older man who is in a vegetative state due to a severe brain injury. The story is set on a remote wooded island, where the only inhabitants are Joslyn, Frank, and a few other caretakers who come and go occasionally.

As days turn into weeks and weeks into months, Joslyn's isolated routine begins to take a toll on her mental and emotional state. The lack of human connection and constant solitude start to affect her psyche, causing her to question her own identity and desires.

One day, while exploring the island, Joslyn stumbles upon a hidden journal belonging to Frank. Through reading his heartfelt and intimate thoughts, she becomes deeply fascinated by his inner life and begins to develop a strange emotional attachment to him. This connection blurs the boundaries between caretaker and patient, leading to a forbidden attraction.

Haunted by guilt and battling her own internal demons, Joslyn finds herself grappling with conflicting feelings of compassion, desire, and the moral implications of her growing sexual attraction towards Frank. She is torn between maintaining her professionalism and giving in to her desires. These internal conflicts intensify as Joslyn continues to read Frank's journals, discovering more about his past and personal relationships.

Further complicating matters, Joslyn meets Tom, a charming and attractive local fisherman who sparks her interest. However, her guilt and the weight of her strange relationship with Frank continue to haunt her, causing her to struggle with her feelings for both men and her own sense of self.

As Joslyn becomes increasingly consumed by her conflicting emotions, the line between reality and madness becomes blurred. She begins to experience vivid hallucinations and nightmares, which reflect her inner turmoil and the psychological toll of her isolated existence.

Without delves deep into themes of sexuality, guilt, loss, and the fragile nature of human connection. It explores the complexities of desire, the consequences of unchecked emotions, and the fragility of the human psyche when confronted with isolation and forbidden desires. Ultimately, Joslyn must confront her own demons and find a way to reconcile her conflicting feelings in order to find peace and regain her sense of self.
