In the movie "Maniac," the main protagonist is Frank Zito, a traumatized and disturbed man who suffered severe abuse during his childhood. As a result, he has developed an intense hatred for women. Frank lives a solitary life in New York City, where he owns a mannequin restoration shop.

However, unbeknownst to those around him, Frank's suppressed psychological issues lead him to become a serial killer. He targets young women, particularly those who remind him of his abusive mother, and scalps them as trophies to place on his collection of mannequins.

One day, Frank meets Anna D'Antoni, a talented photographer who takes a particular interest in his mannequins. Anna becomes intrigued by Frank's eerie and haunting creations, unaware of the darkness hidden within him. A unique bond begins to form between them, mainly due to Anna's genuine interest in Frank's artistry.

As their relationship deepens, Frank experiences conflicting emotions. On one hand, he starts to appreciate Anna's beauty and kindness, seeing her as the "perfect woman" who could potentially end his homicidal tendencies. On the other hand, his uncontrollable impulses and inner demons threaten to sabotage this newfound connection.

While struggling with his instincts and desires, Frank tries to resist his urge to kill and instead focuses on building a genuine relationship with Anna. However, as his psychological state deteriorates and the pressures of his traumatic past intensify, his ability to control his violent tendencies becomes increasingly difficult.

As the story unfolds, the audience witnesses Frank's internal struggle, exploring the themes of trauma, obsession, and the conflicting desire for human connection. The movie oscillates between Frank's attempts at redemption and his relentless descent into madness. Whether Frank will ultimately find solace or succumb to his homicidal tendencies forms the basis of the tension-filled plot.
