After spending two decades in a mental asylum for brutally murdering her unfaithful husband and his lover with an axe, Lucy Harbin (played by Joan Crawford) is finally released. Upon her release, Lucy returns to her estranged daughter, Carol (played by Diane Baker), who was just a small child when her mother was convicted.

Carol, now a young woman, is apprehensive about Lucy's return and skeptical about her mother's sanity. Despite her doubts, Carol tries to reconnect with Lucy, hoping to rebuild their fractured relationship. Lucy, determined to start afresh and prove her sanity, moves into Carol's home.

However, as Lucy settles into her new life, strange occurrences begin to take place. People connected to Lucy's past start turning up dead, seemingly killed by someone wielding an axe. The murders eerily resemble the ones that landed Lucy in the asylum all those years ago.

The town soon becomes consumed by fear, and whispers of Lucy's culpability resurface. The authorities, including a determined detective (played by George Kennedy), begin investigating the crimes.
