In the film "Before I Hang," the story revolves around Dr. John Garth, a respected physician who is sentenced to death for performing a mercy killing on a terminally ill patient. While waiting on death row, Dr. Garth seizes the opportunity to continue his research on a serum that he believes has the potential to reverse aging.

With permission from the authorities, Dr. Garth acquires the blood of recently executed criminals to experiment with his serum. However, consumed by curiosity and a desire to accelerate his research, he secretly administers the serum to himself.

Unexpectedly, Dr. Garth's experimental treatment is successful. Not only does the serum reverse his aging process, but it also unlocks a split personality within him, resembling the classic Jekyll-and-Hyde trope. During the day, he remains the kind and compassionate Dr. Garth, while at night, his alter ego emerges as a violent and sinister version of himself.

Soon, Dr. Garth discovers that he has been granted a last-minute pardon and is released from death row. Thrilled to have evaded execution, he resumes his medical career but struggles to keep his alter ego under control. Night after night, the sinister side of him continues to grow stronger, with violent tendencies that Dr. Garth can no longer suppress.

As the movie progresses, Dr. Garth's allies, including his daughter, become increasingly suspicious of his erratic behavior. Simultaneously, a series of murders occur in the city, with evidence pointing towards Dr. Garth's alter ego as the culprit.

Dr. Garth desperately attempts to find a solution to his split personality predicament before it completely consumes him. He seeks the help of fellow scientists, delving deeper into the secrets of his serum and attempting to understand the nature of his transformation.

Ultimately, Dr. Garth faces a race against time to find a cure for his Jekyll-and-Hyde condition and prevent his alter ego from committing more heinous acts. The film escalates with intense psychological and moral dilemmas, as Dr. Garth battles with his dual nature and the consequences of his experimentation.

The conclusion of "Before I Hang" explores whether Dr. Garth can find redemption, control his alter ego, and ultimately maintain his sanity amid the chaos that his serum has unleashed.
