Tetsuo: The Iron Man follows the story of a salaryman named Tetsuo who starts experiencing strange occurrences in his life. One day, he accidentally runs over a mysterious metal fetishist with his car and flees the scene. From that point on, Tetsuo's life takes a dark turn as he begins transforming into a grotesque hybrid of flesh and metal.

As the transformation progresses, Tetsuo's body starts to incorporate various scrap pieces of metal, leading to an increasingly surreal and nightmarish existence. His relationship with his girlfriend also becomes strained as she witnesses his bizarre transformation and struggles to understand what is happening to him.

Meanwhile, the metal fetishist, who somehow survives the accident, also undergoes his own transformation. Fuelled by his obsession with merging flesh and metal, the metal fetishist seeks revenge on Tetsuo and embarks on a maniacal mission to completely assimilate him.
