"The Villainess" follows the life of Sook-hee, a young girl who is trained as an assassin since childhood in the Yanbian province of China. After witnessing the murder of her father, she is raised by a group of criminals who train her to become a skilled killer. Sook-hee gains exceptional combat skills, blending into any environment seamlessly.

The opening scene sets the tone for the film as Sook-hee embarks on a brutal mission, killing numerous foes without hesitation. However, following a shocking twist, Sook-hee is captured by the South Korean government agency and offered a deal. They promise her freedom after ten years of service as an agent under a fake identity, working for a secret organization known as KFO.

Accepting the offer, Sook-hee undergoes extensive plastic surgery and a new identity as Cha Yeon-soo. In her new life, she moves to South Korea and starts attending school, hiding her true identity as an assassin.

While trying to find a sense of normalcy, Sook-hee becomes romantically entangled with two men. The first is Hyun-soo, a kind-hearted neighbor who falls in love with her despite her mysterious past. The second is Joong-sang, a fellow agent who becomes her mentor and love interest. Both men are unaware of Sook-hee's actual background, adding tension and complexity to her personal life.

As Sook-hee continues her double life, carrying out missions for the KFO, her past catches up with her. Secrets are unveiled, and her true identity comes under the spotlight threatening the lives of those around her. Sook-hee's skills are put to the test as she initiates a violent path to protect herself and those she cares about.
