Milo is a young teenager living in a low-income housing project in New York City. He is dealing with the recent loss of his older brother, who was killed in a random act of violence. As a way to cope with his grief and loneliness, Milo becomes fixated on vampire mythology and immerses himself in horror films, books, and articles. He starts to believe that he himself is a vampire.

One day, Milo meets Sophie, a young girl who also lives in the same housing complex. Sophie is struggling with her own issues, including an abusive father and feelings of isolation. Despite their differences, Milo and Sophie form a deep connection; they understand each other's pain and find solace in one another's company.

As their friendship grows, Milo and Sophie spend most of their time together. They explore the abandoned and decaying parts of the city, sharing their darkest secrets, fears, and desires. Milo, however, becomes more and more convinced that he needs to feed on human blood to become a true vampire.

Milo begins stalking random strangers, observing them in great detail and exploring his violent fantasies. He documents these potential victims on a wall in his apartment, keeping a grim tally of his eerie preoccupations. Sophie witnesses some of these disturbing behaviors, but rather than being frightened, she tries to understand Milo's turmoil and support him as best she can.

As Milo's fantasies and reality merge, he starts to blur the line between his obsession with vampires and his own violent tendencies. The film emphasizes that Milo's fascination with vampirism is not meant to glamorize violence but rather to symbolize the darkness and isolation of his own existence.

However, the consequences of Milo's actions become increasingly apparent. His thirst for blood leads him down a dangerous path, where his fantasies might turn into horrifying reality. Ultimately, The Transfiguration explores themes of trauma, the power of friendship, and the struggle to escape the darkness within oneself.
