In the not-so-distant future, the world is facing an ecological crisis caused by global warming and rising sea levels. To combat the population decline, Cybertronics introduces highly advanced humanoid robots called Mechas, designed to assist humans in various tasks. Among them is David (Haley Joel Osment), a robotic boy created to love and be loved.
Monica (Frances O'Connor) and Henry Swinton (Sam Robards), a couple whose biological son Martin (Jake Thomas) is in a coma, decide to adopt David as a temporary replacement. Monica hesitates at first but eventually grows to love David like her own child. However, when Martin unexpectedly recovers, David becomes an outsider in the family, no longer needed or wanted.
Unable to accept the reality that he might be abandoned, David sets out on a dangerous journey to become a "real boy" and find the mystical Blue Fairy, who he believes can grant his wish. Accompanied by Gigolo Joe (Jude Law), a pleasure Mecha on the run, David travels through a haunting and perilous world.
As David encounters various human and robot societies in his search for the Blue Fairy, he experiences both compassion and cruelty. Along the way, he discovers a place called Rouge City, an urban center filled with debauchery and excess, where humans and Mechas interact in unconventional ways. He crosses paths with various characters, some of whom help him and others who exploit him for their own purposes.
Eventually, David learns about a place called "the end of the world," where all obsolete Mechas are destroyed. Desperate to avoid this fate, he meets Dr. Hobby (William Hurt), the founder of Cybertronics, who reveals that he can grant David a unique opportunity. Dr. Hobby explains that for David to be truly alive, he needs to experience one moment of genuine love from a human.
In a heartbreaking and surreal climax, David realizes that his one true chance of being loved is from Monica. The Swintons, all realizing David's innocence and love, make a difficult choice that will forever change their lives. Through a series of bittersweet events, David is granted one perfect day with Monica, before willingly entering a deep slumber, hoping that one day, advanced humans or Mechas will emerge who can awaken and understand him.
