In Saw VI, Detective Mark Hoffman continues to carry out the legacy of the deceased Jigsaw killer. The film opens with a gruesome trap where two loan officers are tested by having to choose who among them will live and who will die. After their deaths, a message implicates the company they worked for, the same company that denied John Kramer's (Jigsaw) health insurance, leading to his eventual demise.

As the FBI grows suspicious of Detective Hoffman's involvement in the Jigsaw murders, Agent Peter Strahm's death is uncovered, making Hoffman the prime suspect. Meanwhile, a new game is set in motion by Hoffman, targeting a number of individuals connected to the corrupt insurance company. The game focuses on their moral decisions regarding who deserves to live or die based on their involvement in denying legitimate healthcare claims.

One of the primary targets is William Easton, an executive at the company who is known for his ruthless denial of insurance claims. As the game progresses, William must face various deadly traps that force him to confront the consequences of his actions. Meanwhile, the FBI begins closing in on Hoffman, and Agent Erickson becomes increasingly suspicious of his colleague's involvement.

Simultaneously, Hoffman discovers a hidden room that Jigsaw had prepared, revealing his grand plan. It becomes clear that Jigsaw intended for Hoffman to be tested and face his own choices, just as the previous victims had. Hoffman realizes that, in order to survive and maintain Jigsaw's legacy, he must outsmart both the FBI and those seeking revenge for his actions.

In a shocking twist, it is revealed that Jill Tuck, Jigsaw's ex-wife, is working with a secret accomplice. Together, they plan to expose Hoffman's true nature and ensure that he will face justice for his crimes. As the game reaches its climax, Hoffman finds himself in a deadly confrontation with Jill and the secret ally, leading to a final confrontation that will determine the future of Jigsaw's legacy.
